Free Diabetes Risk Assessments

  • ONLINE Diabetes Risk Assessment
    Click on the link above to take an online diabetes risk assessment to determine your risk for diabetes without a fingerstick. Please note that this test is NOT diagnostic and is only a measure of relative risk.

  • FINGERSTICK Diabetes Risk Assessment.
    DAGC's Diabetes Health Educator conduct FREE Diabetes Risk Assessments (including blood sugar testing with a glucose finger stick test) at DAGC's office:

    Appointment required. Call (0.
    A parent or legal guardian must accompany minor children (under the age of 18) to sign a consent form before a risk assessment can be performed. You should fast from midnight prior to being tested. CLICK HERE for directions.
