Assistance with your diabetes supplies
In addition to offering diabetes self-management classes and medical nutrition therapy classes for clients, we also offer aid to individuals who have financial difficulties. They may receive necessary supplies for blood glucose testing and insulin administration, including a blood glucose meter, test strips, insulin syringes and alcohol swabs.
A request for a one-time emergency supply of insulin and syringes must be accompanied by a prescription.
Diabetes Partnership of Cleveland is the sole agency in Northeast Ohio offering an emergency insulin program that provides emergency insulin for individuals who experience a sudden disruption in prescription coverage. We utilize grant monies to pay for the insulin through a pharmacy. When grant monies run out we depend on community supporters.
Insulin is funded through donations. We cannot guarantee that you will receive insulin from Diabetes Partnership of Cleveland. Requests that are granted will be shipped to your home or the location of your choice Monday through Friday by Exact Care Pharmacy and will be followed up by a pharmacy representative.
Diabetes Partnership of Cleveland does not keep insulin on the premises. Requests made after 3:00pm on Friday will be delivered the following Monday. You must be present by phone at the location. A signature is required for delivery.
If you are in need of insulin and you have none, please report to your medical provider or the recommended urgent care center or emergency room immediately!
If you have diabetes, but do not have medical insurance OR your insurance does not cover diabetes supplies, we can help!
Our Community Outreach Specialist can help clients navigate healthcare coverage (Medicare, Medicaid and the Health Care Martetplace), patient assistance programs and medication discount programs.
If you need insulin syringes, we will need a prescription from your doctor. We will not provide syringes unless we have a prescription. You can bring the prescription with you or you can have your doctor fax the prescription to Diabetes Partnership of Cleveland at 216-591-0320. When we receive the fax, we will call you and tell you when your supplies are ready to be picked up at our office located at 3601 S. Green Rd., Suite 100 in Beachwood.
We cannot help you with:
- Diabetes medicines (other than one-time emergency supply of insulin)
- Money to pay your expenses
Here is how the assistance program works:
Call us at 216-591-0800 and ask to talk with our community outreach specialist. Please have the following information ready. The community outreach specialist will ask you:
- Your name, address and phone number
- If you have health insurance
- Do you have a meter? Name of the meter:______
- How often do you test your blood sugar?
- What are the names of the diabetes medicines you take?
Pick up your supplies:
Directions: We are located south of the intersection of Green Road and Chagrin Boulevard on the 1st floor of the Fairways Building located at 3601 S. Green Road, Suite 100.
Please do not come to the office to pick up supplies until the community outreach specialist tells you they are ready.
We are not able to mail supplies.
We are not able to deliver supplies.
If you need a one-time emergency supply of insulin please have the following information ready:
Your name, address and phone number
Social Security number
The name of your insurance
Name of your insulin(s)? How much?
Do you need syringes or pen needles?
Location of your prescription (Pharmacy name and phone number)
